Baldur's Gate 3 is packed with companions, quests, and unique items that quietly disappear if you advance the story without triggering them. This interactive checklist covers every confirmed missable per act — toggle a tab, check items off as you go, and your progress is saved automatically in your browser. 02Act 1 — What You Can Miss and Why

Act 1 is the largest and most open act, but it has a hard stop: the moment you cross the bridge into the Shadow-Cursed Lands for Act 2, you cannot return to the Wilderness. Every companion, quest, and item on the Act 1 tab must be resolved before that transition.

Companions: Wyll is easiest to miss — he stands inside the Emerald Grove near the training dummies. Karlach is north-east on the Risen Road; letting her paladin hunters kill her or raiding the Grove before talking to her locks her out. Withers is not a combat companion but unlocks free respec from camp, which matters enormously for optimising builds.

Unique events: Auntie Ethel's Hair is the single most potent permanent upgrade in Act 1 — you must reduce her HP below 20% in battle before she flees, at which point she offers it as a bribe. Skipping this costs you a permanent +1 to any ability score. Playing a musical duet with Alfira at camp is a missable scene — she is absent if you raid the Grove or fail to save the Tieflings — so trigger it before crossing into Act 2.

Volo chain: Rescue Volo from the Goblin Camp (he's in a cage near the performance area) before completing 'Save the Refugees.' He moves to your camp and later performs crude eye surgery, giving you Volo's Ersatz Eye — a permanent See Invisibility effect that works all game.

Items: The Everburn Blade is only obtainable in the Nautiloid prologue by using Command: Drop on Commander Zhalk and looting it from his body. Once the prologue ends, it is gone forever. Phalar Aluve requires either Strength 18 or a Religion 15 check to pull from the rock in the Underdark — an easy miss if you rush past it.

03Act 2 — The Shadow-Cursed Lands

Act 2 is shorter but more tightly scripted around Last Light Inn and Moonrise Towers. Several companions and major quest rewards expire if you advance the Moonrise Towers storyline too quickly.

Last Light Inn pivots: Defending Isobel when Marcus attacks the inn is the single most consequential event in Act 2 — if Isobel dies or is taken, the Inn falls, the surrounding area loses its protection, and Jaheira becomes hostile or is killed. She cannot join your party without surviving that fight. Rolan also rushes out to face shadows near the Inn shortly after this — help him immediately or he dies and his siblings' rescue quest in Moonrise Towers loses its conclusion.

Minthara: Only available if she was knocked unconscious (not killed) in Act 1. Find her in the Moonrise Towers Prison alongside the Tiefling prisoners. Rescue her before assaulting the rooftop.

Gauntlet of Shar: Shadowheart's choice at the Nightsong at the end of the Gauntlet defines her entire Act 3 arc and ending. The Dark Justiciar armour inside the Shadowfell is only obtainable during this quest — it cannot be bought or looted elsewhere.

Lift the Shadow Curse: Halsin enters the Shadowfell to retrieve Thaniel as a prerequisite to fully lifting the curse. This quest must be completed before you leave for Act 3. If you advance to Moonrise Towers rooftop without doing it, the questline becomes inaccessible.

04Act 3 — Baldur's Gate City

Act 3 has the most missables because the city contains two hard deadlines tied to boss kills — killing Gortash or Orin locks out several quests and NPCs permanently.

Stop the Presses: The most time-sensitive quest in Act 3. After speaking with the editor-in-chief at Baldur's Mouth Gazette about the smear campaign, you have one long rest to complete the quest. If you kill Gortash first, the quest disappears entirely, meaning you permanently miss key story information and the associated items.

Iron Throne order: The Steel Watch Foundry and the Iron Throne Prison must be done in the right order. If you destroy the Foundry first, the Steel Watchers active at the Iron Throne will massacre the Gondian prisoners and Duke Ravengard before you can rescue them. Always rescue the Iron Throne prisoners first, then destroy the Foundry.

Minsc: Only recruitable if Jaheira survived Act 2 and is in your party when you encounter him in the Counting House sewers. Minsc then joins permanently after a battle. If Jaheira died in Act 2, this entire questline is closed.

Companion personal quests: Astarion's Cazador questline and Shadowheart's House of Grief quest (finding her parents) must both be completed before the final assault on the Netherbrain. After you commit to the endgame, unfinished companion quests are permanently abandoned.

Legendary items: Several best-in-slot weapons are easy to miss — Markoheshkir is in the lower level of Ramazith's Tower (not the upper floor where the fight happens), Nyrulna requires winning the jackpot on the Djinni Ring at the Circus of the Last Days, and Balduran's Giantslayer requires completing the Wyrmway dragon trials beneath Wyrm's Rock.

FAQ

What is the point of no return between Act 1 and Act 2 in BG3?

Crossing the bridge into the Shadow-Cursed Lands is the one-way threshold between Act 1 and Act 2. The game warns you before you cross. Once in Act 2, you cannot return to the Wilderness, Underdark, or Goblin Camp — every Act 1 companion, quest, and item must be resolved before this transition.

Can you miss companions in BG3?

Yes. Several companions are missable: Wyll and Karlach can both be locked out by certain Act 1 decisions (raiding the Grove, or letting Wyll/Anders kill Karlach). Minthara requires knocking her out in Act 1, not killing her. Halsin is lost if you leave Act 2 without completing 'Lift the Shadow Curse.' Jaheira is lost if Isobel dies at Last Light Inn. Minsc requires Jaheira to be alive and in your party in Act 3.

What happens if you kill Gortash too early in Act 3?

Killing Gortash before completing 'Stop the Presses' makes that quest permanently unavailable. You also lose access to certain Steel Watch-related content. The general advice is to explore all of Baldur's Gate city — including the Foundry, the Gazette, and Wyrm's Rock — before fighting any of the three antagonists (Gortash, Orin, Ketheric was Act 2).

Is Minthara always missable on a good playthrough?

Minthara is recruitable without raiding the Emerald Grove — you can knock her unconscious during the Goblin Camp encounter in Act 1, and she wakes up in Moonrise Towers Prison in Act 2. This requires passing a specific check. The non-lethal path still locks some Tiefling content but avoids the full evil raid. Details are patch-dependent, so verify against current Patch 8 notes.

Can you miss Auntie Ethel's Hair in BG3?

Yes — her hair (+1 to any ability score) is one of the most powerful permanent upgrades in Act 1 but is very easy to miss. You must reduce Auntie Ethel's health below 20% during combat in the Sunlit Wetlands / Putrid Bog. At that point she offers the hair as a bribe to let her flee. If you kill her outright without triggering that threshold, or if you spare her without fighting, you do not receive it.

What is the best order to do Act 3 quests in BG3?
